Tongue in Cheek

Patient: I offered to help my little sister with her homework the other day but she turned me down.

Doctor: Did you speak tongue in cheek?

Patient: I do not get it.

Doctor: Tongue in cheek is a figure of speech to imply that a statement is expressed ironically. People stick their tongue against the inside of their cheek after saying something to show that it is not serious but in fact just a joke.
